We are seeking a compassionate and organized Hospice Services Coordinator to coordinate referrals, admissions, scheduling, documentation, and communication with patients, families, referral sources, physicians, and our clinical team. This role serves as an important liaison, helping ensure timely communication and coordination of hospice services.

What You’ll Do 

As a Hospice Services Coordinator, you will:

  • Coordinate and maintain a consistent hospice intake process in accordance with established policies, procedures, and workflows.
  • Receive and process referrals by phone, fax, and electronic portal; create referral records in the EMR and attach all required supporting documentation.
  • Request additional records or information as needed and communicate referral updates and pending information to the appropriate team members.
  • Collaborate with Directors, Managers, and clinical team members regarding referral acceptance, scheduling needs, and other patient-care coordination.
  • Provide exceptional customer service by educating patients, families, referral sources, and other customers about hospice services and responding to inquiries in a professional and compassionate manner.
  • Document calls, communications, follow-up, and outcomes accurately and timely within the appropriate systems.
  • Coordinate patient scheduling and visit assignments based on patient needs, ordered services, clinical direction, staff availability, and geographic considerations. Communicate schedule changes with patients, caregivers, and field staff as needed.
  • Prepare, process, and track physician orders, face-to-face documentation, and other required clinical documentation; follow up with physicians and physician representatives to obtain completed and signed documents and support regulatory compliance.
  • Maintain accurate tracking systems and documentation to support compliance with applicable federal and state hospice requirements.
  • Provide administrative support to the Hospice Bereavement Program, including post-death follow-up, staff notifications, documentation, EMR updates, bereavement packets, scheduled follow-up calls, and record closure.
  • Assist the Bereavement Counselor and management with memorial services and other bereavement-related activities, including monitoring bereavement volunteer activity.
  • Support the hospice volunteer program, including volunteer referrals, recruitment, onboarding, training, scheduling, assignments, engagement, and retention; maintain accurate volunteer files, credentials, hours, and EMR documentation.
  • Support Veterans Affairs recognition initiatives, including preparation of dog tags, welcome letters, Quilt of Valor activities, and other veteran-focused programs.
  • Work collaboratively with the hospice management and interdisciplinary teams to improve customer satisfaction, strengthen internal processes, provide coverage as needed, and support overall hospice operations.

What You’ll Need 

  • High school degree required, college degree preferred, or equivalent years of training or work experience.
  • Minimum of one year health care/business experience in a fast-paced, customer service environment.
  • Strong communication and problem-solving skills.
  • Planning and organizing skills to achieve personal and business unit goals.
