As an RN Case Manager, here’s a big-picture view of what you’ll do:
- Develop and update individualized Plans of Care (POC) based on client needs, physician orders, and agency policies.
- Ensure timely reassessments and supervisory visits per state and agency requirements.
- Serve as a liaison among physicians, clients, families, and other interdisciplinary team members.
- Participate in quality assurance activities, chart audits, and staff meetings.
- Coach family members on the patient’s plan of care, medication and home safety
- Communicate changes in condition, medication, or care needs promptly to the appropriate parties.
- Maintain accurate and timely documentation in the electronic health record (EHR) system.
A few must-haves for Registered Nurses:
- Graduate of an accredited nursing program and active RN license in Virginia
- Minimum of (2) Two years of RN nursing experience, ideally in home healthcare
- CPR certification
- Knowledge of state and federal home health regulations
- Clinical proficiency in nursing specialty, strong communication skills and compassion
