Interim Healthcare is looking for an LPN for our Home Health Department. The territory is Kansas City/Independence/Raytown and surrounding areas if needed.

As a Home Health Licensed Practical Nurse, here’s a big-picture view of what you’ll do:

• Recognize and report changes in the patient’s condition to the RN supervisor.
• Complete timely and accurate clinical notes including addressing patient’s progress.
• Assist the RN or physician in performing specialized procedures and duties.
• Assist the RN in carrying out the patient plan of care.
• Assist the client in learning appropriate self-care techniques.
• Able to perform CPR if needed.
• Follows accepted standards of nursing practice.
• Communicate with referral sources to identify home health needs.
• Enters patient demographic information on all home health patients into the appropriate database.
• Able to travel within geographic area serviced by office from assignment to assignment.
• Complete other assignments as requested and assigned.
• May have access to protected health information (“PHI”) necessary to fulfill the above duties and responsibilities. Access to use and ability to disclose PHI is further defined by each organization.

A few must-haves for Home Health Licensed Practical Nurses:

• Must have MO and KS or compact license.
• Must be willing to work in KS and/or MO.
• Home Health experience is preferred, but not required.
• Must have CPR certification.
• Must have wound care experience.
• Must have at least one year of nursing experience.
