Salary Range: $38.00-$53.20 per hour

Compensation decisions are dependent on the facts and circumstances of each case and on several factors including relevant work experience, education, certification & licensure, and internal equity. Job Summary

   Supervisory Duties:

  • Continuously uphold the highest degree of performance within currently defined and acceptable nursing practice and infection prevention standards.
  • Maintaining an effective friendly working relationship with all guests, vendors, consultants, and government agencies.
  • Assist DON/Administrator in analysis of department needs, studies trends and developments and offers appropriate suggestions.
  • Authorize schedule changes effectively with over-time hours and level of staff in mind.  Make changes on schedule so verification of hours worked can be done. Plans daily assignments with team members based on resident acuity, staff qualifications, continuity of care and infection control.
  • Coordinate daily assignments of care effectively.
  • Lead weekly interdisciplinary team meetings, attend Medicare meetings, and participate in weekly High Risk meetings to ensure effective communication through the interdisciplinary team regarding resident care and changes.
  • Participate in rounds with physicians or medical care providers as needed
  • Transcribes and carries out physician orders in an accurate and timely fashion as needed
  • Communicates pertinent resident/facility information, changes of resident function or need clearly to team members from all departments and all shifts as needed.
  • Review NAR, RN and LPN charting for completeness and accuracy.
  • Report Concerns for NAR, RN and LPN to DON/Administrator
  • Provide day to day leadership and supervision of nursing staff and nursing activities.
  • Resolve problems, and complaints and report them to the Director of Nursing.
  • Report unusual situations, supervisory problems or other areas of concern to DON. May provide input for performance evaluation of nursing staff.
  • Understand and maintain compliance with Medicare coverage of skilled nursing care.
  • Ensure that the facility’s policies and procedures governing advance directives are reviewed with the resident and /or representative.
  • Function as orientation preceptor for new nursing staff.
  • Participate in mandatory educational opportunities and staff meetings, complete all required online education.
  • Comply with standards established by Quality Assurance and Performance Improvement Committee

 

BRIEF SUMMARY OF RESIDENT CARE DUTIES:

  • Develop, implement and maintain ongoing quality insurance programs for resident assessments and care plans.
  • Provides appropriate care accurately according to the plan of care.
  • Ensure that a baseline resident centered care plan is completed within 48 hours of resident admission.
  • Ensure that a comprehensive resident centered care plan is completed within 21 days of resident admission and is kept current.
  • Ensure that quarterly and annual resident assessments are care plan reviews are made on a timely basis.
  • Communicate changes in resident function or needs clearly and in a timely fashion. Ensure that a complete resident assessment is conducted within 14 days of a significant change in the resident’s condition and reports significant information to physician and resident’s family.
  • Ensure that medical and nursing care is administered in accordance with the resident’s wishes, including the implementation of advance directives and discussion of advanced directives quarterly.
  • Provide adequate charting to maintain optimal RUG classification data of residents/clients and document resident’s/client’s problems, nursing approaches, care plan accurately and according to charting schedule.
  • Demonstrate ability to assess and evaluate resident needs in regards to maintenance and restorative therapy as recommended by PT, OT, ST or physician.
  • Develop and implement the maintenance therapy portion of the resident care plan, integrate into nursing care plan.
  • Organize facilitate resident’s care conferences quarterly.
  • Monitor charting by therapy professionals for timeliness, accuracy, optimal resident outcomes, and highest reimbursement potential.
  • Assist with direct resident care related to activities of daily living.
  • Conduct initial and periodic comprehensive, accurate, and standardized assessments of each resident.
  • Ability to initiate, coordinate and document resident care/support
  • Coordinates resident centered discharge planning as needed upon discharge or transfer to another facility, provides legible, accurate and pertinent information.
  • Coordinates in resident/family teaching
  • Act as resident advocate and protect resident confidentiality.
  • Able to operate equipment involved in resident care, keep current with new operating, maintaining and cleaning existing and new equipment.
  • Initiate and Complete Vulnerable Adult Reports as needed when on-call or Social Worker is not available

 

Qualifications

  • A nursing degree from an accredited college or university,
  • Minimum two (2) years of experience providing direct care in long-term care, restorative or geriatric setting as an LPN/RN.
  • Minimum (1) year of experience in nursing management.
  • Current CPR Certification.
