At Visiting Angels Columbus West, we’re seeking a mission-driven RN Care & Program Manager to lead the development, oversight, and execution of our home care clinical programs. This role is ideal for a nurse who thrives in a collaborative environment and is passionate about care quality, caregiver training, and supporting older adults to live with dignity and independence.

Why This Role Matters

As our RN Care & Program Manager, you will ensure the delivery of high-quality, person-centered care through active oversight of care plans, caregiver education, and client wellness programs. You’ll also help shape the resident experience at our new assisted living home by overseeing activities that promote engagement, purpose, and connection.

What You’ll Do

Client-Centered Clinical Oversight

• Complete in-home assessments and develop individualized care plans based on client needs and preferences
• Monitor and adjust care plans as conditions evolve or needs change
• Serve as a key point of contact for clients and families with questions related to care delivery and clinical needs

Caregiver Training & Program Development

• Design and deliver clinical training programs for caregivers in alignment with client needs and home care best practices
• Serve as a mentor and clinical resource to the caregiving team, ensuring competence and confidence in field delivery
• Contribute to the ongoing development of specialized care programs (e.g., dementia care, fall prevention, palliative support)

Quality, Compliance & Engagement

• Ensure regulatory compliance with all state and federal guidelines related to nursing and home care
• Conduct field visits and performance audits to assess care delivery quality and ensure caregiver adherence to care plans
• Oversee the creation and execution of resident activities and wellness programs at the assisted living facility, promoting physical, cognitive, and emotional engagement
• Support a holistic care environment by working with staff to design group and individualized activities that foster purpose, routine, and community among residents

Cross-Team Collaboration

• Collaborate with the Director, Client Care Managers, and Scheduling team to support client intake, transitions, and continuity of care
• Participate in interdisciplinary meetings and provide clinical input to support decision-making and team alignment

What We’re Looking For

• Current Ohio RN license in good standing
• Minimum 3 years of experience in home care, hospice, long-term care, or related community-based nursing setting
• Strong interpersonal skills with a passion for supporting seniors and their families
• Demonstrated ability to lead training and coaching for direct care staff
• Experience or interest in planning and implementing resident activities or wellness programs in a care setting
• Familiarity with home care documentation platforms and strong organizational skills
• A proactive mindset, clinical curiosity, and a genuine desire to improve outcomes and quality of life
• Reliable transportation and ability to conduct home visits throughout the Columbus area
