About the Role:
We’re seeking a compassionate and organized Resident Care Coordinator to support day-to-day operations in our senior living community. This role ensures high-quality, person-centered care by coordinating resident services, supporting care staff, and collaborating with physicians, families, and interdisciplinary team members.
As a key leader within the care team, you will help foster a safe, supportive, and engaging environment for our residents while ensuring compliance with health standards and best practices.
What you'll do:
- Lead and support care team operations in partnership with the Director of Resident Care.
- Coordinate and communicate effectively with physicians, staff, and department heads.
- Ensure proper staffing based on census and state requirements.
- Oversee resident health needs, including vitals assessment and care planning.
- Conduct medication cart audits and ensure accurate MAR documentation.
- Participate in on-call and weekend rotation.
- Coordinate move-ins, assessments, and family care conferences.
- Support hiring, onboarding, and training of care staff and medication aides.
- Monitor resident files, complete audits, and maintain state compliance.
- Step in as a caregiver or med passer when needed.
- Create and maintain individualized care plans.
- Foster a positive and educational work culture through ongoing coaching.
- Collaborate on daily rounds and follow-ups with residents and families.
What we're looking for:
- High School Diploma or equivalent required.
- Prior experience in assisted living, long-term care, or a similar healthcare setting preferred.
- Must be caring, observant, and reliable.
- Strong communication and leadership skills.
- Comfortable with flexible scheduling, including weekends or on-call shifts.
- Ability to read, write, and speak English fluently.
- Familiarity with electronic medical records and MARs a plus.
Physical Requirements:
- Ability to bend, lift, and use proper body mechanics.
- Frequent walking, sitting, and repetitive hand motions.
