Hospice Field Nurse

University Home Care Inc is seeking a compassionate and dedicated Hospice Field Nurse to join our team in Wayne County, MI. This role requires a Registered Nurse (RN) with at least one year of hospice experience to provide exceptional end-of-life care to patients in their homes.

Responsibilities

• Provide skilled nursing care to hospice patients in their homes.
• Assess patient conditions and develop individualized care plans.
• Coordinate care with interdisciplinary teams to ensure comprehensive patient support.
• Educate patients and families on symptom management and end-of-life care.
• Document patient care accurately and timely in compliance with company and regulatory standards.

Requirements

• Valid Registered Nurse (RN) license.
• Minimum of 1 year hospice nursing experience.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Being able to travel if needed 
• Ability to work independently and manage time efficiently.
