Skilled Home Health LPN

Responsibilities

• Provide skilled nursing care and treatment to patients in their homes according to physician orders and care plans.
• Monitor patient health by taking vital signs, assessing symptoms, and evaluating progress.
• Administer medications, injections, and treatments as prescribed.
• Coordinate with physicians, therapists, and other healthcare professionals to ensure comprehensive patient care.
• Educate patients and their families about health maintenance and disease prevention.
• Document patient information and care provided accurately in compliance with company and regulatory policies.
• Maintain patient confidentiality and adhere to ethical standards of nursing practice.

Requirements

• Valid LPN license in the state of Michigan.
• Previous experience in home health nursing preferred.
• Familiar with Kinnser 
• Strong communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Compassionate and patient-centered approach to care.
• Reliable transportation to visit patient homes.
