Responsibilities

• Provide direct nursing care to patients in their homes according to care plans.
• Administer medications and treatments as prescribed by healthcare providers.
• Monitor and document patient condition and progress accurately.
• Collaborate with healthcare team members to ensure comprehensive care.
• Educate patients and families on health management and care techniques.
• Maintain all necessary documentation and comply with regulatory requirements.

Requirements

• Valid Licensed Practical Nurse (LPN) license in the state of Michigan.
• Prior experience in home health or related nursing field preferred.
• Strong interpersonal and communication skills.
• Ability to work independently and manage time effectively.
