Licensed Practical Nurse (LPN)

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team at University Home Care Inc. In this role, you will provide essential nursing care to patients in their homes, ensuring they receive the highest quality of care in a comfortable and supportive environment. You will work closely with healthcare providers and families to assist with treatment plans and promote patient well-being.

Responsibilities

• Provide skilled nursing care to patients in their homes according to established care plans.
• Monitor and record patient vital signs and report any significant changes to the care team.
• Administer medications and treatments as prescribed by physicians.
• Assist with wound care and other clinical procedures as needed.
• Educate patients and families on healthcare management and disease prevention.
• Maintain detailed and accurate patient records and documentation.
• Collaborate with other healthcare professionals to coordinate patient care.

Requirements

• Valid LPN license in the state of Michigan.
• Previous experience in home health or related nursing field is preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Compassionate and patient-centered approach to care.
