Responsibilities

• Provide skilled nursing care to patients in their homes, including assessment, treatment, and medication administration.
• Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
• Monitor patient progress and adjust care plans as necessary to ensure optimal health outcomes.
• Educate patients and their families on health conditions, treatments, and self-care techniques.
• Document all patient interactions, treatments, and outcomes accurately and timely according to company standards.
• Coordinate with physicians and other healthcare professionals to provide comprehensive patient care.
• Maintain patient confidentiality and adhere to all regulatory and compliance standards.

Requirements

• Current Registered Nurse (RN) license in the state of Michigan.
• Previous experience in home health nursing visits or related field preferred.
• Excellent clinical assessment, communication, and organizational skills.
• Ability to work independently and manage a flexible schedule.
• Reliable transportation to travel to patients’ homes within Livonia, MI.
