University Home Care Inc is seeking a dedicated Homecare Physical Therapist with Kinnser experience to provide exceptional in-home physical therapy services to patients in Southfield, MI and the surrounding areas. This role requires a compassionate professional able to deliver personalized care plans that support patient mobility and recovery in a home setting.

Responsibilities

• Conduct in-home physical therapy evaluations and treatments in accordance with established standards.
• Develop and implement individualized treatment plans focused on improving patient function and quality of life.
• Document all patient interactions and treatment progress accurately using Kinnser software.
• Communicate effectively with patients, families, and healthcare team members to coordinate care.
• Maintain compliance with regulatory requirements and company policies.
• Travel to patient homes within Southfield and surrounding areas as required.

Requirements

• Valid physical therapy license in the state of Michigan.
• Experience with Kinnser homecare software required.
• Proven ability to work independently and manage time efficiently.

• Reliable transportation and willingness to travel to patient homes.
