Hospice Social Worker

University Home Care Inc is seeking a compassionate and dedicated Hospice Social Worker to join our team in Livonia, MI. In this role, you will provide essential support and resources to patients and their families during end-of-life care, helping to ensure dignity, comfort, and emotional support throughout the hospice journey.

Responsibilities

• Provide counseling and psychosocial support to hospice patients and their families.
• Conduct assessments to identify patient and family needs, challenges, and resources.
• Coordinate community resources and referrals to support patient care.
• Serve as an advocate for patients and families to ensure their wishes are respected.
• Document patient interactions and care plans accurately and timely.
• Maintain accurate and timely documentation in accordance with company policies and regulatory requirements.
• Participate in team meetings and contribute to quality improvement initiatives.

Requirements

• Master’s degree in Social Work (MSW) from an accredited program.
• Current Licensed Social Worker (LSW or LCSW) in the state of Michigan.
• Prior experience in hospice or palliative care social work preferred at least 1 year.
• Strong communication, interpersonal, and organizational skills.
• Ability to work independently and as part of a multidisciplinary team.
