University Home Care is currently seeking a Home Care Intake Coordinator to join our team. This position plays a vital role in providing excellent customer service and coordinating home care services for our clients. Responsibilities:

  • Process incoming referrals and coordinate initial client assessments
  • Collect and verify all necessary client information
  • Ensure accurate and timely coding of client records
  • Coordinate with healthcare professionals to develop and revise client care plans
  • Schedule and coordinate home care services based on client needs and availability of caregivers
  • Answer phone inquiries from clients, caregivers, and healthcare professionals
  • Maintain electronic and paper documents in compliance with regulations and company policies

Requirements:

  • Prior experience in a healthcare or home care setting
  • Excellent communication and customer service skills
  • Strong organizational and documentation skills
  • Ability to work in a fast-paced environment
  • Proficient in Microsoft Office and relevant software applications
  • Knowledge of medical terminology and coding
