University Home Care is currently seeking a Home Care Intake Coordinator to join our team. This position plays a vital role in providing excellent customer service and coordinating home care services for our clients. Responsibilities:
- Process incoming referrals and coordinate initial client assessments
- Collect and verify all necessary client information
- Ensure accurate and timely coding of client records
- Coordinate with healthcare professionals to develop and revise client care plans
- Schedule and coordinate home care services based on client needs and availability of caregivers
- Answer phone inquiries from clients, caregivers, and healthcare professionals
- Maintain electronic and paper documents in compliance with regulations and company policies
Requirements:
- Prior experience in a healthcare or home care setting
- Excellent communication and customer service skills
- Strong organizational and documentation skills
- Ability to work in a fast-paced environment
- Proficient in Microsoft Office and relevant software applications
- Knowledge of medical terminology and coding
