Part-Time Post-Acute Care Physician – Brady, TX (1 Day/Week)

Trumen Physicians + Associates is seeking a compassionate and experienced Physician to join our post-acute care team in Brady, TX for one day per week. About the Role:
The physician will provide in-person medical care to a panel of patients in a skilled nursing facility in Brady, TX. This includes management of acute and chronic conditions, care transitions, and collaboration with the facility’s interdisciplinary team and Trumen’s internal care coordination staff.
Key Responsibilities:

Perform in-person visits and follow-up care for assigned patients

Collaborate with facility nursing staff and Trumen’s medical directors

Update care plans, manage medications, and ensure continuity of care

Participate in value-based care and transitional care initiatives

Complete documentation in a timely manner within the EMR
Qualifications:

MD or DO with board certification in Internal Medicine, Family Medicine, or Geriatrics

Active and unrestricted Texas medical license
-
At least 1 year of clinical experience, preferably in post-acute, SNF, or geriatric care

Strong communication and organizational skills

Reliable transportation to/from facility in Brady, TX
