TITLE: Assisted Living Clinical Coordinator
EXEMPT STATUS: Non-Exempt
REPORTS TO: Assisted Living Director
SUPERVISES:Certified Nursing Assistants, and Certified Medication Aides
GENERAL SUMMARY:
The AL Clinical Coordinator is responsible for the overall supervision, provision, and quality of nursing care and member services at Heather Glen Assisted Living. He/she is responsible for the selection, training, supervision, and development of all member care personnel at Heather Glen Assisted Living. He/she is responsible for the procurement of appropriate supplies and equipment and operating within the departmental budget. The Coordinator provides the overall positive leadership for all direct care staff with a focus on exemplary customer service, the highest standards of member care, and teamwork.
QUALIFICATIONS:
- Graduate of an accredited school of nursing. LPN with a current North Carolina license.
- Two years experience in geriatric nursing and/or Bachelor of Science in Nursing preferred. Two years nursing administration experience, preferably in a long term care setting.
- Basic computer proficiency.
- Strong interpersonal and communication skills.
Essential Job Duties:
- Responsible for the development, organization and operation of nursing services and other member services for Heather Glen assisted living.
- Coordinates all specialty services for Heather Glen residents including professionals such as home health care, laboratory, x-ray, oxygen, hospice, podiatry, psychological and physicians necessary to ensure that resident’s health needs are met.
- Creates and directs a staffing plan to assure 24-hour coverage sufficient to meet State regulatory requirements, Heather Glen standards of care, and budget guidelines. Maintains records of daily staff assignments for a minimum of one year.
- Trains and evaluates all Health Service personnel and develops action plans when needed.
- Completes a functional assessment for each resident prior to moving into Heather Glen
- Ensures compliance with all state regulations pertaining to member care for NC Adult Care Homes.
- Assures compliance of federal HIPPA privacy standards and conducts annual training for all employees.
- Serves "on-call" on weekends and after normal working hours to provide clinical direction to the healthcare staff as needed
- Ability to communicate effectively with residents, families and staff. Strong organizational skills are a plus
