We are seeking a compassionate, dependable, and experienced Licensed Practical Nurse (LPN) to join our Home Health team. The LPN provides skilled nursing care to patients in their homes under the direction and supervision of a Registered Nurse (RN) and in accordance with the established plan of care.
Responsibilities

Provide skilled nursing care to patients in their homes according to the established plan of care.

Monitor and document patients’ conditions, vital signs, symptoms, and progress.

Administer medications and treatments as ordered by the physician and permitted within the LPN scope of practice.

Perform wound care and other skilled nursing procedures as assigned.

Observe patients for changes in condition and promptly report concerns to the supervising RN or appropriate clinical team member.

Educate patients and caregivers regarding medications, treatments, safety, and the plan of care.

Maintain accurate, timely, and complete clinical documentation.

Communicate effectively with the RN, physicians, caregivers, and other members of the interdisciplinary care team.

Follow all agency policies, infection-control procedures, patient-safety standards, and applicable state and federal regulations.

Travel to patient homes within the assigned service area.

Job Requirements
- Current, valid Florida Licensed Practical Nurse
- Graduate of an accredited physical therapy program
- Minimum of two years of physical therapy work experience
- Current CPR/BCLS certification
- Current health certificate, physical exam, and TB clearance
- Valid driver's license, reliable vehicle, and current auto insurance; ability to pass a criminal background check
Qualifications
- Strong clinical judgment with the ability to synthesize assessment findings into an effective care plan
- Excellent verbal and written communication skills for patient education and interdisciplinary coordination
- Ability to work independently while managing a home visit schedule efficiently
- Compassionate, patient-centered approach to home health and rehabilitative care
- Strong organizational skills and attention to documentation detail
- Prior home health experience preferred
- Bilingual candidates a plus
