Qualifications: 

  1.  LVN or RN license required. 

  2. Maintain a current Texas nursing license. 

  3. It is the responsibility of each nurse to keep up his or her own required  continuing education hours. 

  4. Health and Wellness Director must maintain a current CPR certification. 

General Job description: 

The Health and Wellness Director is responsible for the overall implementation,  delivery, and coordination of resident services at the community. Directly supervises  all employees of the nursing department. This position is responsible for the overall  direction, coordination, and evaluation of this department. This person also carries  out supervisory responsibilities in accordance with the organization's policies and  applicable laws. The responsibilities include training employees; planning, assigning,  and directing work; appraising performance; rewarding and disciplining employees;  addressing complaints and resolving problems. 

Essential Job Functions: 

  1. Serve as liaison between facility, physicians and third-party vendors  concerning resident change of condition, change in medication, and other pertinent  information regarding health status. Communicate with family member/legal  guardian regarding health status, change of condition, etc.

  2. Medication management to include, oversight and verifying medications  according to doctor’s orders and availability ensured in a timely manner for  medication administration. 

  3. Medications are to be documented and charted on in EMAR according to State  Guidelines. Obtain and enter resident demographic information in to EHR. Ensure  resident orders are entered and accurate according to physician’s orders. Assign  resident services either prior to or upon resident admission. 

  4. Re-order resident medications from pharmacy per physician’s order.  Pharmacy will deliver or families will pick-up prescriptions and deliver to their  Enriched Senior Living facility.

  5. Obtain and/or complete resident health forms, to include the PPOC, H&P,  Service Plans, comprehensive assessments, AL level of care, wellness, and skin  assessments. The Executive Director may initiate the completion of forms. 

  6.  Timely complete assessments according to state regulation and community  policy and procedure.

  7. Maintain a First-Aid Kit, as is guided by Regulations. Materials should include  needs for minor cuts, lacerations and burns. 

  8. Be familiar with Assisted Living regulations especially pertaining to nursing  services and medications. 

  9. Obtain physician signature on consolidated medication and treatment orders  quarterly. 

  10.  Assess a potential resident in their home, another facility or at the community.  Upon management request, be willing to travel on rare occasions. 

  11. Obtain and document resident vital signs once each month. Includes weight,  temperature, pulse and respiration (any abnormalities are reported to physicians).  Assist caregivers with periodic assessments of residents, some lifting may be  required. 

  12. Assess and discuss all residents on a routine bases to ensure services provided,  care plan and care levels match resident needs. Implement resident care needs in a  timely manner and with the Executive Director discuss any changes with resident and  or responsible party.

  13. Ensure timely, open and honest communication with residents and their  responsible party. Inform responsible party and obtain consent of any change or new  physician orders. 

  14. On call 24 hours (phone) for any staff questions/concerns. 

  15.  Participate in staff training each month, as necessary. Also, participate in new  employee orientation and training.

  16. Offer basic first aid training to staff. Serve as guidance in general assessments  in the event of a resident fall.

  17. Manage expenses within budgetary guidelines.

  18. Maintain confidentiality of all resident’s information. 

Line of Authority: 

The Health & Wellness Director reports to the Executive Director.
