The Registered Nurse will be responsible for client care, and will be expected to evaluate clients’ needs, prepare and sign-off care plans, and ensure assigned caregivers provide appropriate care. Additionally, the RN will also be responsible for training and evaluating caregivers’ skills, verifying skills, and providing training and support during onboarding orientation.

Senior Helpers North Metro services clients located in Andover, Anoka, Coon Rapids, Champlin, Circle Pines, Elk River, Ham Lake, Lino Lakes, Otsego and Ramsey.

Objective: To oversee the care of clients and monitor aides, perform nursing assessments, and write, monitor, and update care plans for clients with a home care agency.
Reports to: Owner and/or Director of Operations

Primary Responsibilities (including, but not limited to):

  • Coach all staff in proper safety procedures and consistently reinforce procedures
  • Train and evaluate caregivers during orientation to provide guidance and knowledge around proper techniques and guidelines for caregiving as it relates to the job, while also serving as the subject matter expert for the hands on skills assessment required for caregivers
  • Travel to and from clients' homes and complete all necessary documentation for nursing assessments and observations for clients in all parts of the territory listed above
  • Plan and execute evaluations pertaining to clients and caregivers
  • Evaluate clients on a regular basis and/or as necessary to ensure client needs are consistent with the care plan
  • Coordinate and collaborate with management on implementing and revising operational procedures related to quality client care
  • Utilize professional assessment skills to identify physical, mental, and psychosocial needs of client and be able to develop a care plan to meet these needs in a multi-disciplinary approach
  • Function as a resource and mentor to peers and subordinates to maintain professional nursing standards
  • Act on client’s behalf to ensure patient rights
  • Be aware of home safety issues, strive to accomplish all responsibilities in a safe manner, and consistently reinforce good safety attitude and practices displayed by other staff members
  • Document any client complaints concerning a caregiver and review complaints with the management team

Qualifications:

  • Able to work as a team member - this is critical to the success with Senior Helpers North Metro
  • At least one (1) year of home health care experience
  • Technological experienced - not limited to; scheduling software, Microsoft Outlook and other digital programs to streamline business practices. 
  • Able to maintain confidential information
  • Able to communicate effectively with company staff, the clients, and client’s families
  • Able to supervise (instruct, assign work, etc.) caregivers while on duty
  • Excellent verbal and communication skills, adaptable in different situations, and possess excellent client interaction skills
  • Able to multi-task and work independently
  • Required Education and Experience:
  • ADN or BSN
  • Current, unencumbered RN license
  • CPR Certification
  • Current TB Test
  • Must maintain CEU requirements as required by the Board of Nursing
  • Valid Driver's License and reliable transportation

Preferred Qualifications:

  • Have at least five (5) years of home health care experience
  • Training and supervisory experience
  • Experience working with VA patients/clients
