Senior Helpers of East Brooklyn is seeking a compassionate and dedicated nurse for care planning, caregiver skills training, and field operations. Preferred but not required, candidates should be multilingual in English and other languages.

 

Responsibilities

  • Client care planning, reassessment every 6 months or less, compliance reporting.
  • Coordinate care with team, clients, families, and medical professionals.
  • Assess client health status and report significant changes to healthcare providers.
  • Caregiver skills training at Center of Excellence.
  • Enforce safety, infection control, and healthcare regulations.
  • Maintain accurate and timely documentation of care provided and deliverables.

 

Requirements

  • Valid RN or NP New York State License.
  • Valid license, registration, insurance for reliable personally owned vehicle.
  • Experience in gerontological care in community, facility, or both settings.
