Position Summary: Under limited supervision, the STARS Case Manager is responsible for providing services for all STARS participants. The STARS Case Manager, in the absence of the Senior Director of Operations and Housing, oversees day-to-day operations, including policies and procedures, operational goals, reporting requirements, and regulatory compliance. The STARS Case Manager is responsible for ensuring services to empower participants to address economic, child care, employment, education, health, housing, transportation, life skills, parenting, recovery, federal, state and local assistance, vocational assistance, GED referrals and family needs, in addition to identified substance use and mental health issues to facilitate and assist in their transition to self-sufficiency, stability, and recovery. The STARS Case Manager is responsible for keeping abreast of resources available by establishing and maintaining relationships with outside agencies, which offer medical and mental health services, permanent housing services, employment services, educational services and social services. The STARS Case Manager must provide tracking data for sustainability of quality case management services. No smoking workplace
Essential Duties and Responsibilities include the following. Other duties may be assigned.
- Provides outreach and engagement services to individuals and community partners to identify potential participants and bring them into services
- Meets with participants and assists with developing an individualized recovery/service plan
- Provides mentoring/individualized transition and recovery support assistance.
- Guides participant through community integration to assist with finding/maintaining housing, educational opportunities, volunteer activities, and obtaining employment. Collaborates with community providers to link participant to these services.
- Establishes and maintains relationships with community agencies and stakeholders
- Facilitates recovery support groups and other recovery-oriented activities.
- Collaborates with other recovery support organizations, aftercare providers, churches, corporations, alumni, and volunteer organizations.
- Ensures program is compliant with all OVC/housing standards, regulations, and guidelines.
- Ensures new residents are orientated to their new housing and understand their handbook policies and procedures.
- Ensures there is no significant disruption in supportive services to residents.
- Works with Senior Director of Operations and Housing to meet operation targets in housing
- Ensures privacy and confidentiality of participants
- Provides data collection and reporting through enrollments, census, client surveys, and recovery wellness groups.
Education/Experience:
A bachelor’s degree in Social Work or a related field is required for this position, or equivalent combination of education and experience in the field. Previous experience in recovery, case management/coaching and crisis intervention required. Knowledge of special needs of the target population, substance use and ethical standards required.
Certificates and Licenses:
LBSW or LMSW preferred
Approval Requirements:
Position may require approval from Texas Department of Criminal Justice (TDCJ) to work in TDCJ-funded programs, if applicable.
