Salus Hospice is urgently seeking a compassionate and skilled Hospice Admissions RN to join our dedicated team serving the Orange County region. This position will cover the following areas: All of Orange County with the ability to cover South Bay, LA as needed

This is a WEEKENDS ONLY Position: Scheduled Saturday and Sundays 10 AM -8:30 PM

Hospice Admissions RN Responsibilities:

• Participation in the information gathering, analysis, education and form completion aspects of the admissions process

• Evaluate patients for hospice appropriateness, e.g. medical records review as requested.

• Obtain agreement for admission of referred patients and families who meet admission criteria.

• Build effective referral source relationships to support the high quality and responsiveness expectations associated with Salus Hospice

• Maintain clinical and professional competence.

• Utilize interpersonal skills to help appropriately educate and address any questions the patient and/or patient’s family has concerning hospice care.

Hospice Admissions RN Qualifications:

• Registered Nurse with valid CA state license

• Minimum one year of hospice or home health experience

• Strong background in medical/surgical, ICU/CCU, or ER is helpful

• Valid driver’s license, auto insurance, reliable transportation and willingness to drive to patient’s place of residence

• Must have excellent interpersonal skills and a passion for hospice

• Understanding of hospice philosophy and needs of the terminally ill

• Self-directed with ability to work with little supervision

• Current Physical and TB test results
