In collaboration with the client, family and caregiver, the patient care manager assesses, plans, and implements the delivery of private home care and reassess the situation to act in the client’s best interest.

The role of Patient Care Manager is an in-office management and administrative nursing position reporting directly to the Director of nursing.      

Nurse Management 

  • Management and oversight in the areas of client care, staff and caregiver supervision, compliance, and overall client satisfaction and well-being;
  • Respond to client referrals and service inquiries in a timely and efficient manner, driving the inside sales process by consulting with prospective families and overseeing the process through to home assessment, client intake and service delivery;
  • Perform a multi-dimensional home assessment and collect data concerning the client’s values, past history, physical and emotional health, cognitive status, coping abilities, economic resources, and environmental conditions;.
  • Determine client’s requirements based on assessment and develop a personalized care plan that balances the client’s needs and the appropriate available resources. The care plan, developed in collaboration with the client and family, encompasses priorities, holistic well-being, and short and long term goals;
  • Establish a relationship with the client honouring the client’s values and preferences;
  • Work closely with the scheduling and administrative team to ensure optimal caregiver matching and the highest levels of customer service;
  • Reassess clients at regular intervals to monitor changes and update the care plan as required to ensure optimal care delivery;
  • Assess risks and identify changes in condition that would facilitate care plan adjustments and communication with the family;
  • Ensure accurate and timely completion of client charting, reporting, and all relevant documentation;
  • Help facilitate the growth of Saige through professional referrals, community networking, identifying opportunities, and providing an exceptional client experience;
  • Management of projects aimed at achieving business, educational, and service delivery goals as directed.

Staff Development & Administration

  • Supervise, schedule and manage overall performance of operational and caregiving staff, in cooperation with the Director of nursing;
  • Ensures staff development including caregiver orientation, in-service education, continuing education and evaluation of staff;
  • Facilitate client/staff introductions and oversee staff preparation for new clients. Guide and direct staff in the performance of their duties;
  • Achieves/exceeds employee satisfaction and investigates issues and follows up promptly;
  • In office documentation, general administration, and reporting as required.

Education/Experience Requirements

  • Registered Nurse maintaining active registration with CRNA.
  • Minimum two (2) years of clinical experience with at least half of that time working in home health or related health care organization;
  • Minimum two (2) years of direct employee supervision;
  • Possess exceptional assessment and client consultative skills;
  • Excellent customer service skills and ability to deal tactfully with clients, staff and community;
  • Must be proficient with computers, software systems, and Microsoft Office;
  • Excellent written and verbal communication skills;
  • Valid driver’s license and reliable vehicle required.

Work Environment

  • The role of patient care manager is an integral part of the Saige Homecare team. This is a full time in-office position in SE Calgary with occasional travel to client homes for home assessments and check-ins.
  • The PCM will participate in an on-call rotation for after hours client/staff concerns.
  • This is a supervisory and administrative nursing position, having adept office and communication skills is required.
