Location: The Pavilion at Robinson Terrace – Stamford, NY

Schedule: Monday to Friday, 8:00am to 4:00pm

Are you a compassionate Registered Nurse with strong clinical skills and a heart for seniors? The Pavilion at Robinson Terrace, a trusted assisted living community in the beautiful Catskill Mountains, is seeking a dedicated RN Case Manager to coordinate care, support residents and families, and ensure exceptional health outcomes in a warm, person-centered environment.

Key Responsibilities:

• Oversee and coordinate clinical care for residents in alignment with individual care plans
• Conduct comprehensive assessments and participate in care plan development and reviews
• Monitor resident health status, manage medications, and ensure proper documentation
• Communicate regularly with families, physicians, and interdisciplinary team members
• Provide training and guidance to care staff on clinical procedures and best practices
• Support regulatory compliance and quality assurance initiatives
• Assist in transitions of care and discharge planning as needed

Requirements:

• Active New York State RN license
• Minimum of 2 years of experience in a senior living, long-term care, or related setting
• Strong clinical assessment, critical thinking, and communication skills
• Familiarity with NYS Adult Care Facility regulations preferred
• Experience with dementia care and care coordination a plus
