Care Team Role - Clinical Manager (Director of Nursing)

Job Purpose:
A Director of Nursing provides oversight of all patient care/services and personnel. The Director of Nursing is available at all times during business hours and participates in all activities relevant to the professional care/services furnished. Administrative and supervisory functions are not delegated to another agency or organization.
Qualifications:
- Current state license as a Physician, Physical Therapist, Speech-Language Pathologist, Occupational Therapist, Audiologist, Social Worker, or a Registered Nurse.
- Minimum of two years of home health care experience and at least one year of supervisory experience.
Florida Requirements:
- For Florida licensed skilled home health agencies, the Director of Nursing role functions as the Director of Nursing (DON).
- The Director of Nursing must be a Registered Nurse (RN) licensed in the State of Florida
- The Director of Nursing must have at least one (1) year of supervisory experience as a Registered Nurse
- The Director of Nursing must be a direct employee of the agency as defined under Florida law.
- The Director of Nursing must designate an Alternate Director of Nursing in writing to provide coverage during absences and maintain continuity of clinical oversight.
- The Director of Nursing is responsible for establishing and maintaining the agency’s quality assurance / QAPI program, including documentation of review activities and corrective actions.
Training:
- Experience/training in home health care reimbursement for Medicare and Medicaid preferred.
- Computer skills that include word processing and efficient use of the internet and email.
Compensation:
Part-Time $30 - $40/Hourly Based on Experience
Responsibilities:
- Provides oversight of all patient care/services and personnel.
- The Director of Nursing or a similarly qualified alternate is available at all times during business hours.
- Participate in all activities relevant to the professional care/services furnished, including the development of qualifications and the assignment of personnel.
- All skilled nursing and other therapeutic services are furnished under the supervision and direction of a qualified Director of Nursing.
- The Director of Nursing is responsible for the direction, coordination, and supervision of care/services. The Director of Nursing’s oversight must include the following: » Making patient and personnel assignments.
» Coordinating patient care/services.
» Coordinating referrals.
» Ensuring that patient needs are continually assessed.
» Ensuring the development, implementation, and updates of the individualized plan of care.
- Participates in hiring, orientation, and training of new personnel.
- Ensures adequate staffing of agency to provide quality patient care/services.
- Designates a qualified alternate to assume the same responsibilities and obligations as the Director of Nursing when the Director of Nursing is not available.
- Ensures the agency provides 24-hour coverage by a Registered Nurse and coordinates the on-call schedule.
- Carries out the responsibilities of the Administrator of the agency in the case of his/her absence.
- Coordinates and participates in the agency’s Quality Assessment and Performance Improvement (QAPI) program.
- Identifies and implements changes in clinical and/or operational practice based on findings of the QAPI program and ongoing audits.
- Ensures compliance with regulatory requirements, standards of practice, policies, and procedures.
- Adheres to the agency’s policies and procedures.
- Participates in the annual preparation of the budget.
- Informs the Administrator of personnel, industry, and regulatory changes/trends, as applicable.
- Completes performance evaluations of personnel according to agency policy.
- Coordinates and/or performs competency evaluations for direct care personnel per agency policy.
- Attends and participates in agency education and committee meetings, as appropriate.
- Monitors contract services, if any, to ensure quality of care/services.
- Maintains confidentiality of all aspects of job performance.
- Complies with all applicable federal, state, and local laws and regulations in performance of job functions.
- Performs other related functions as directed.
