Platinum Home Health Services is seeking a compassionate, motivated Physical Therapist (PT) to join our growing home health team. The ideal candidate is committed to providing high-quality, patient-centered care while helping patients improve mobility, function, and overall quality of life in the comfort of their own homes.

Essential Responsibilities:

• Perform comprehensive physical therapy evaluations and develop individualized plans of care
• Provide skilled physical therapy services in accordance with physician orders and agency policies
• Assess patient mobility, strength, balance, and functional limitations
• Implement therapeutic exercises, gait training, and pain-management techniques
• Educate patients and caregivers on treatment plans, safety, and home exercise programs
• Document evaluations, progress notes, and discharge summaries accurately and timely
• Communicate effectively with physicians, case managers, and interdisciplinary team members
• Follow all state, federal, and Medicare home health regulations

Qualifications:

• Current Physical Therapist license in the State of Ohio
• Graduate of an accredited Physical Therapy program
• Home health experience preferred, but not required
• Strong clinical, communication, and organizational skills
• Ability to work independently and manage a caseload
• Valid driver’s license, reliable transportation, and auto insurance
