Position Summary
The PCAH Promise position supports service reliability and client confidence during critical moments in care. This role focuses on coordinating care, stabilizing services, supporting communication with clients and families, and stepping in as needed to prevent missed shifts or service disruptions.
PCAH Promise team members serve as a care coordinator and client experience ambassador, with the ability to provide direct caregiving coverage when necessary. The role is designed to protect client satisfaction, support smooth transitions, and uphold Preferred Care at Home service standards.
Key Responsibilities
Care Coordination & Service Stabilization

Assist with coordinating coverage during scheduling disruptions

Support smooth transitions between caregivers and phases of care

Intervene early when concerns arise to help stabilize services

Communicate updates and observations to the Administrator or Office Manager
Client Experience & Communication

Support new client launches and first visits to establish confidence
-
Communicate professionally with clients and families regarding service concerns
-
Conduct proactive check-ins for at-risk cases as directed

Assist with follow-up after concerns or dissatisfaction
As-Needed Caregiver Support

Provide personal care or homemaker services in accordance with the Plan of Care when assigned

Serve as a short-notice fill-in to prevent missed or delayed shifts

Model professionalism, safety, and respectful client interaction
Operational Support

Document continuity notes to support caregiver handoffs

Identify and report patterns affecting service reliability or satisfaction
-
Support office follow-up actions related to service recovery
Documentation & Compliance

Accurately record time and services using EVV and agency systems
-
Complete visit notes, incident reports, and required documentation timely

Maintain confidentiality and comply with HIPAA and privacy requirements
Qualifications
-
At least one year of caregiving experience (CNA, HHA, CHHA, PCA, or equivalent)

Strong communication and interpersonal skills

Ability to remain calm and effective in time-sensitive situations

Comfortable working in both client homes and with office staff

Reliable transportation and smartphone proficiency
Work Schedule & Conditions

May include evenings, weekends, holidays, and short-notice assignments

Travel between multiple client homes may be required
