The work of a Direct Support Worker is directed by a plan of care developed by the Oregon Department of Human Services. The direct support worker follows the individual service plan for each individual.

NORTH PORTLAND
Client 1:
Location: Portland 97217 | Age 10yrs Male | Schedule: Mon- Fri- Varies and weekends | Loves his pet dog, school, legs puzzles, music, iPAD, and his family. Would like a person that is into gaming, like ROBLOX. Needs to be confident, energetic, creative, persistent, and consistent. Good listener with lots of patience, be physically faster than client, he can run fast, keep him safe from dangerous situations, and work on taking Logan into the community
Client 2:
Location: North Portland 97203 | 21yrs Male | Schedule is open depending on DSPS and Clients needs| Most shifts available | Wants support with assisting him in home and community support. Relies heavily on DSP for physical assistance, particularly with transfers, dressing, and personal call, and requires, vigilant support during meals due to choking hazard

Location: North Portland 97229 | 12yrs Female | Schedule: Time vary during week, weekends 4-5 hours | Likes to swim, loves music and singing, puzzles and books.

Location: North Portland 97229 | 8yrs Male | Schedule: Times vary during the week, weekends 4-5 hours | Likes books puzzle, Youtube, sun and stars. Sibling to client 3
NORTH EAST PORTLAND
Client 5
Location: North East 97220 | Schedule: Tue 345-7pm, Thur 345-7pm, Fri 730am-8:30am ( 8 mile ride from home to JC Portland) Fri 345-7pm, Sat any 3-6 hr shift. Sin any 3-6 hr shift, Every other weekend is okay too. Client is flexible with hours outside of Job Corp | Client is admired for her intelligence, sweetness, and kindness, alongside a tenacity for personal growth and success. She is recognized for her strong communication skills, particularly in her school environment, and for being a resilient individual who perseveres through challenges | Key supports identified for client encompass individualized assistance in social and physical activities, ensuring she remains engaged and continues her journey toward independence and self-advocacy. In her support plan, Client aspirations include learning to navigate her community more independently, focusing on developing job skills in medical coding through Job Corps, and strengthening her physical and communication abilities.
Client 6:
Location: North East 97212 | 22yrs Female | Schedule: Tue- Fri 2pm-7pm and Sat a few hours | Likes: Songs, to talk about what she likes | Prefers someone her own age, and likes the same thing she likes needs support with cuing for restrooms and general support

Location: North East 97212 | 6yrs Male | Schedule: 345pm- 8:30pm, Wed, Thur- Sun 9am-830pm Flexible | Likes: loves to smile, be active and move around a lot. Loves time with family and friends, loves people, loves learning, does not like loud noises
Client 8:
Location: North East 97233 | Schedule: 530am-8am Mon, Tue, Thur, Fri and no Wed from 530am-9am | Likes: Plays video games and watches TV in the morning, toys basketball, bikes, scooters | Needs: Assistance getting ready for school. School is about 5 minutes from the home.
NORTH WEST PORTLAND
Client 9:
Location: NW Portland 97210 | 11yrs Female | Schedule: Mon- Thur 230pm-6:00pm, Fri 12:00pm-6:00pm | Likes: Helping her mom and dad, cooking is important to her, she likes making toast and smoothies, Likes music, singing, eating, playing piano, choir, playing soccer, and biking. Likes baby dolls and can dress them. She is always on the run | Needs: Needs prepping for transitions, lots of structure, she would like to be busy with extracurricular. Benefits from visual schedules and really needs to whats next. Choices are important to her
Client 10:
Location: NW Portland 97229 | 18 yrs Male | Schedule: Fri 2pm-9:30pm, Sat and Sun 5-8 hr shifts, between 9am-9pm | Likes: Enjoys structure and routines | Needs: Support with community outings, toileting and mealtime independence
Client 11:
Location: NW Portland 97229 | 17yrs Male | Schedule : Tue-Thu 4pm-8pm , weekends, and if wanted respite shifts | Likes: Client is a fun, inquisitive individual who enjoys computer games, Legos, and social activities with peers. His sense of humor and bright smile make him well-liked by those around him | Needs:Client is supported by his providers in achieving these goals, with an emphasis on clear communication, structured routines, and community engagement

Location: NW Portland 97229 | 15yrs Male | Schedule: Tue- Thur 4pm-8pm, weekends, and if want respite as well | Likes: He enjoys gaming, values community activities that involve his peers and family | Direct Supports play a key role in helping him navigate transitions, understand community dynamics, and build confidence in his personal abilities.
SE PORTLAND
Client 13:
Location: SE Portland | 13yrs Male | Schedule: Mon-Tue (but could make it all week if DSP wanted) 3pm-5pm, Sat 9am-3pm | Likes: He loves HVAC Systems and anything involving the chef store or known as cash and carry | Needs: Someone to support client after school until family comes home.
Client 14:
Location: SE Portland 97233 | 10yrs Female | Schedule: Mon- Fri 4:00pm-8:00pm | Likes : To draw and cut things out that she draws, she likes TV | Needs: Supports after school when it is more difficult. Needs help making her use the bathroom. Would like to get her into the community and help her communicate with others and build a support group
Client 15: SE Portland 97266 | 8yrs Male | Schedule: 1:00pm- 5:00pm A few days a week and weekend if possible | Likes: to build, active, needs attention and to make people smile | Needs: Willingness to be firm yet fun, able to adapt to an active little boy, likes to run, must be okay with dogs.
Client 16: SE Portland 97206 | 7yrs Female | Schedule: Mon- Fri 3:30pm- open, Weekends open and flexible on days and times | Likes: Loves blueberries, her family, likes her own room and be able to control her environment. Likes time to recharge | Needs: Social stories and practice conversations starters, develop friendships with other kids.
Client 17: SE Portland 97202 | 7yrs Male | Schedule: Tue, Wed, and Thur 2:30pm-5/5:30pm | Likes: His mother, books are very important to him. he has a creative ,mind, thinking about numbers and patterns are kind of important. Client likes to think about future projects and goals and really likes Chess | Needs: Like things to be predictable and have a routine, being an engaged listener and meeting him where he is at. Positive redirection, never yelling, Offer alternatives and create a Plan B, Employ humor and story time. No Power struggle.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Deliver training, support, and assistance to the clients in the areas of personal care, recreation and leisure, daily living skills, social skills, health needs, personal preferences, exercising of individual rights, community integration, and behavior management.
- Participate in the implementation and monitoring of the clients individualized support plan. Your observations and opinions are crucial to tailoring an achievable plan.
- Accurately complete required documentation including but not limited to clocking in and out, weekly progress notes, and communication with the family of the client and the office staff.
- Ensure clients health and safety at all times including but not limited to prevention of abuse, perform CPR, perform Standard First Aid, and other needs that the client may stand in need of.
- Work directly with the client/family of the client to ensure the goals of the client are being met and being worked towards
- Cooking, cleaning, medication assistance, scheduling, attend medical appointments, shopping, and more...
- Skill development and support is defined in the individual’s Individual service plan
- Documentation of progress notes with each shift
Qualifications and Education Requirements:
- High school diploma or equivalent
- Personal Support: 1 year previous experience (required)
- Transporting individuals
- Be certified in CPR/First Aid upon being hired
- Overall competency in working under pressure
- Experience in providing general care and support
- Knowledge of home care services
- Proficient in written communication
- Proficient in oral communication
- Be at least 18 years of age
- Understand requirements of maintaining confidentiality and safeguarding individual information.
- Be literate and capable of understanding written and oral orders.
- Be able to communicate with individuals, health care providers, case managers, and appropriate others.
- Be able to respond to emergency situations at all times services are being delivered.
- Must have a valid Drivers License, Auto Insurance, and reliable transportation in compliance with the laws of the Department of Motor Vehicles. (Proof and copies)
