MDS Coordinator

A role as an MDS Coordinator is available at Mountain Ridge Health & Rehab. As an MDS Coordinator, you will be responsible for coordinating the Minimum Data Set (MDS) assessment process for patients and ensuring compliance with state and federal regulations. This role requires detailed understanding of MDS regulations and the ability to accurately complete assessments.

• Coordinate and complete MDS assessments accurately and timely
• Ensure proper documentation and coding of MDS assessments
• Ensure compliance with state and federal regulations related to MDS assessments
• Collaborate with the interdisciplinary team to develop individualized care plans
• Participate in meetings and trainings related to MDS assessment processes

• Valid nursing license
• Experience with MDS assessments
• Knowledge of state and federal regulations related to MDS assessments
• Attention to detail and ability to accurately complete assessments
