About the Position

The Chesapeake, a LifeSpire of Virginia community in Newport News, is seeking a dedicated Memory Care Nurse Supervisor to join our Assisted Living team.

The Memory Care Nurse Supervisor is responsible for supporting the overall well-being of memory care residents through the delivery of high-quality nursing services in accordance with professional nursing standards and applicable federal and state regulations.

Working closely with the Assisted Living Nurse Manager, this position provides clinical leadership and oversight within the memory care neighborhood, promotes quality resident care, and supports team members in providing compassionate, person-centered services to our residents.

Key Responsibilities

Clinical Leadership & Resident Care

  • Support the mission, vision, and values of LifeSpire of Virginia.
  • Assist the Assisted Living Nurse Manager in ensuring high-quality nursing services are provided to Assisted Living and Memory Care residents.
  • Provide clinical oversight and coordination of physician services within the Memory Care neighborhood.
  • Review the 24-hour report daily and follow up on pending resident care or operational concerns.
  • Process physician orders and track follow-up on laboratory results, X-rays, and special procedures.
  • Complete resident assessments upon admission, leave of absence, and following falls.
  • Coordinate new resident admissions and level-of-care transfers as needed.
  • Develop and maintain ISPs, UAIs, and fall and risk assessments for Memory Care residents.
  • Complete weekly skin assessments and work collaboratively with RMAs and CNAs to monitor and address resident skin conditions.
  • Provide and/or coordinate appropriate wound care and ensure preventative measures are implemented.
  • Oversee fall prevention and management programs.
  • Complete investigations related to incident reports and implement appropriate interventions.
  • Attend monthly resident care plan meetings with residents and families as part of the interdisciplinary team.

Medication & Treatment Management

  • Conduct monthly medication and treatment cart reviews.
  • Coordinate medication destruction and routine medication refills.
  • Collaborate with the Nurse Manager on annual updates to the medication management plan.
  • Support nursing staff by covering the medication cart during breaks, callouts, and as needed.

Communication & Quality Improvement

  • Promote effective communication regarding resident care and services.
  • Address resident and family concerns in a timely and professional manner.
  • Ensure appropriate nursing staff consistently utilize resident care plans.
  • Promote effective communication within and between nursing shifts.
  • Maintain and protect the confidentiality of resident information at all times.
  • Identify trends in resident care and develop corrective action plans as needed.
  • Participate in Risk Management and QAPI meetings.
  • Ensure compliance with federal and state regulations related to nursing services, as well as organizational policies and procedures.

Team Leadership & Staffing

  • Provide supervision, coaching, and counseling to team members.
  • Develop effective staffing and scheduling patterns that promote continuity of resident care across all shifts.
  • Ensure the nursing needs of Memory Care residents are appropriately met.
  • Participate in the on-call rotation for higher levels of care.
  • Promote positive, constructive working relationships among staff and across departments.

Collaboration & Program Support

  • Develop and maintain professional working relationships with outside physicians, pharmacy consultants, dietitians, and therapy partners.
  • Support the Program Manager in providing therapeutic recreational activities and resident engagement opportunities.
  • Assist the Administrator with the development and revision of nursing policies and procedures.
  • Maintain professional competence through participation in required annual continuing education.

Compliance & Professional Standards

  • Follow all personnel policies and procedures outlined in the Employee Handbook and subsequent organizational publications and notices, including policies regarding breaks, meal periods, and PAL time.
  • Maintain thorough knowledge and understanding of:

Qualifications

  • Current Licensed Practical Nurse (LPN) license issued by the Commonwealth of Virginia Board of Nursing.
  • Two years of gerontological nursing experience.
  • UAI, ISP, and CDP certification, or ability to obtain certification within six months.
  • Strong organizational and time-management skills.
  • Ability to provide compassionate, high-quality nursing services to older adults.
  • Demonstrated ability to effectively supervise and support a diverse team.
  • Excellent oral and written communication skills.
  • Strong interpersonal skills with the ability to work effectively with residents, families, team members, healthcare providers, and other professionals.
  • Ability to maintain a positive, constructive, and collaborative working environment.
