Care Manager – Licensed Social Worker (Aging in Place Concierge Care)

We’re seeking a compassionate and resourceful Licensed Clinical Social Worker (LCSW) or experienced MSW to provide high-touch care management services that make a lasting difference.

In This Role, You Will:

Conduct comprehensive psycho social assessments and client intakes

Develop and implement personalized care plans to support aging in place

Serve as a trusted liaison between clients, families, physicians, and care providers

Advocate during transitions of care (e.g., hospital discharges, facility placements)

Facilitate family meetings, coordinate community resources, and monitor client well-being

Collaborate closely with our RN, LVN, and caregiver teams

What You Bring:

LCSW (preferred) or MSW with geriatric, hospice, or medical social work experience

Deep knowledge of aging-related concerns, local resources, and psycho social support strategies

Excellent documentation, communication, and crisis management skills

Empathy, flexibility, and strong boundaries
