In this role, you’ll be the champion of care continuity—connecting patients to specialty care, managing vital medical records, tracking lab results, and ensuring no patient falls through the cracks.

What You’ll Do
- Care Coordination & Patient Navigation (40%): Lead referral workflows for specialty care, assist patients with outside scheduling, track work queues, and collaborate with providers, pharmacy staff, and community partners.
- Medical Records & Health Information (25%): Serve as our primary lead for incoming/outgoing records, HIPAA-compliant Release of Information requests, and record prepping in Athena EHR.
- Lab & Diagnostic Coordination (15%): Track, schedule, route, and follow up on lab and diagnostic orders to keep care moving forward.
- Operational Cross-Coverage (10%): Support front-office workflows (scheduling, registration, eligibility) during high-volume periods or team absences.
- Mentorship & Workflow Growth (10%): Help train interns and volunteers, refine clinic operating procedures, and participate in quality improvement projects.
What We’re Looking For
- Education: High School Diploma or equivalent required (Degree in Healthcare Admin, Public Health, Social Work, or related field preferred).
- Experience: Background in healthcare, care coordination, patient navigation, or medical records strongly preferred.
- Tech-Savvy: Experience with Electronic Health Records required (Athena experience is a big plus!).
- Skills: Strong organizational skills, HIPAA knowledge, multi-tasking ability, and top-notch communication skills.
- Heart for Service: A commitment to working with diverse populations and advocating for patient needs.
- Bonus Points: Bilingual skills (English/Spanish)!
Work Schedule & Location
- Status: Full-Time
- Format: Hybrid (Regular onsite presence for clinic operations, with remote flexibility for administrative, tracking, and follow-up tasks as approved by the Clinic Manager).
