Physical Therapist (PT) – Home Health

Location: Oklahoma City, OK and Surrounding Areas

What You’ll Do:

• Provide in-home physical therapy services to patients recovering from illness, injury, or surgery

• Develop and manage personalized care plans focused on mobility, safety, and independence

• Collaborate with interdisciplinary team members, including nursing, OT, and physicians

• Monitor progress and adjust treatment plans as needed

• Accurately document visits and communicate with the care team

Requirements:

• Active Physical Therapist license in the state of Oklahoma

• One year of recent clinical experience preferred (home health experience a plus)

• Strong communication, organizational, and documentation skills

• Valid driver’s license, auto insurance, and reliable transportation
