Your Mission (Should You Choose to Accept It):
As our Hospice Clinical Liaison, you’ll be our bridge between the community and our hospice team – educating, inspiring, and guiding families, providers, and partners as they navigate the journey of end-of-life care. You’ll represent a program built on heart, integrity, and excellence, and help ensure that no family faces this season without the support they deserve.

What You’ll Do:

• Build and nurture relationships with hospitals, clinics, SNFs, ALFs, and community providers

• Educate families and professionals on the benefits of hospice care and our unique approach

• Facilitate smooth, compassionate transitions into hospice services

• Represent Interim HealthCare at events, in-services, and outreach opportunities

• Contribute to the growth, visibility, and success of a program that truly matters

What We’re Looking For:

• Licensed Practical Nurse (LPN) – active and in good standing in Oklahoma

• Experience in hospice or home health strongly preferred

• Someone with a natural ability to connect, communicate, and inspire trust

• A self-starter who thrives in the field and is comfortable with light travel

• An advocate, educator, and compassionate representative of our mission
