Interim HealthCare is looking for a motivated, organized, and clinically knowledgeable LPN Home Health Intake Coordinator to join our Home Health team.
This is an excellent opportunity for an LPN who enjoys healthcare but is looking to transition from traditional field-based nursing into an office-based Monday - Friday position. The Intake Coordinator plays a critical role in reviewing referrals, coordinating admissions, communicating with referral sources, and helping ensure patients receive timely access to the Home Health services they need.

As an LPN Intake Coordinator with our Home Health Team, here’s a big-picture view of what you’ll do:
- Serve as a primary point of contact for new Home Health referrals from hospitals, physicians' offices, skilled nursing facilities, assisted living communities, and other referral partners.
- Receive, review, and process incoming Home Health referrals in a timely and organized manner.
- Utilize your clinical knowledge to review referral information and assist in determining patient appropriateness for Home Health services.
- Review diagnoses, clinical information, orders, medications, functional needs, and other relevant information included with referrals.
- Coordinate with the Director of Nursing, clinical leadership, and field clinicians regarding new referrals and potential admissions.
- Obtain missing clinical, demographic, insurance, and referral information necessary to facilitate admission.
- Communicate professionally with physicians, hospitals, facilities, patients, families, referral sources, and members of the clinical team.
- Coordinate timely start-of-care planning and help ensure new patients are appropriately scheduled.
- Verify insurance coverage, benefits, authorization requirements, and other payer-related information as applicable.
- Track referrals from initial receipt through admission or disposition and follow up on outstanding information.
- Identify potential barriers to admission and proactively work with the appropriate team members to resolve them.
- Maintain accurate referral and intake documentation within the electronic medical record and other applicable systems.
- Help ensure referrals and admissions are handled in accordance with applicable regulations, payer requirements, and company policies.
- Develop and maintain strong relationships with referral sources by providing responsive, professional service.
- Assist the Home Health leadership team with improving intake, referral, and admission processes.
- Support the overall growth and success of the Home Health service line through timely and effective referral management.
- Perform other duties as needed to support the Home Health office team.
A few must-haves for the LPN Intake Coordinator role:
- High School Diploma or equivalent required; associate or bachelor's degree preferred
- Excellent communication, organizational, and multitasking skills
- Ability to work in fast-paced environment and collaborate with a multidisciplinary team
- Previous experience in home health, medical intake, scheduling, or healthcare administration
- Knowledge of Medicare, Medicaid, and Private Insurance guidelines
- Proficiency with EMR/EHR systems and Microsoft Office Suite
