What do we offer?

  • Licensed Social Worker Pay Rate: $45-$50 per hour, paid weekly

Position Summary

The Licensed Social Worker (LSW) is responsible for providing comprehensive psychosocial support to residents in a skilled nursing facility. This role ensures compliance with federal and state regulations while promoting resident well-being, dignity, and quality of life through assessment, care planning, and discharge coordination.


Key Responsibilities

  • Complete comprehensive psychosocial assessments upon admission, quarterly, annually, and with significant changes
  • Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team (IDT)
  • Lead and participate in care conferences with residents and families
  • Coordinate discharge planning, including transitions to home, assisted living, or other levels of care
  • Provide counseling, emotional support, and crisis intervention to residents and families
  • Ensure compliance with CMS regulations and F-Tags (including F-Tag 679 – Activities, F-Tag 680 – Quality of Life, F-Tag 684 – Quality of Care)
  • Advocate for resident rights, dignity, and person-centered care
  • Assist with advance directives, guardianship, and end-of-life planning
  • Address and resolve resident and family concerns or grievances
  • Collaborate with nursing, therapy, providers, and administration to support behavioral and mental health needs
  • Maintain accurate and timely documentation in EMR systems (e.g., PointClickCare)
  • Support MDS process and documentation as needed

Qualifications

  • Bachelor’s degree in Social Work (BSW) required
  • Active Licensed Social Worker (LSW) in the state of North Dakota
  • Previous experience in long-term care, skilled nursing, or healthcare setting preferred
  • Knowledge of MDS, care planning, and discharge processes preferred
  • Strong understanding of federal and state LTC regulations
