What You'll Do

As a Home Health OT, you will deliver personalized therapy that helps patients participate more safely and independently in daily life:

• Evaluate functional abilities, routines, safety risks, and barriers within the home

• Develop and update individualized therapy plans aligned with physician orders and patient goals

• Provide interventions that improve strength, coordination, endurance, cognition, and daily living skills

• Teach adaptive techniques for bathing, dressing, grooming, meal preparation, and other activities

• Recommend adaptive equipment and environmental modifications that support safety and independence

• Educate patients and caregivers on exercises, strategies, equipment, and the plan of care

• Track progress, adjust interventions, and maintain timely, accurate documentation

• Collaborate with nursing, physical therapy, speech therapy, and other care team members

   

  What You Bring

   

• Graduate of an accredited occupational therapy program with an active Arizona OT license

• Minimum of [insert required experience] of occupational therapy experience

• Home health experience preferred [edit as applicable]

• Current CPR certification

• Valid driver's license, auto insurance, and reliable transportation

• Strong clinical judgment, creativity, communication, organization, and documentation skills
