We are currently seeking a Full-Time Home Health LPN to join our growing clinical team.

Responsibilities

• Provide skilled nursing care in patients’ homes under the direction of an RN/physician
• Monitor and document patient condition and response to treatment
• Administer medications and treatments as ordered
• Educate patients and families on care plans and disease management
• Assist with coordination of care among interdisciplinary team members
• Complete timely and accurate clinical documentation

Qualifications

• Active Oklahoma LPN License
• Current BLS/CPR Certification
• Valid Driver’s License and Reliable Transportation
• Home Health Experience Preferred
• Strong Clinical Assessment and Communication Skills
• Ability to work independently in a home setting
