We are currently seeking a Full-Time Home Health RN to serve patients throughout the Southwest Oklahoma City territory.

Responsibilities

• Complete skilled nursing visits in patients’ homes
• Perform assessments and develop individualized care plans
• Coordinate care with physicians, therapists, and interdisciplinary team
• Educate patients and families on disease management and treatment plans
• Complete timely and accurate documentation
• Monitor patient progress and adjust care as needed

Qualifications

• Active Oklahoma RN License
• Current BLS/CPR Certification
• Valid Driver’s License and Reliable Transportation
• Home Health Experience Preferred
• OASIS Documentation Experience Preferred
• Strong Clinical Assessment and Communication Skills
