Medical Social Worker

At Interim HealthCare of Oklahoma City, we believe care is personal, and so is the support we provide to patients and families. We are seeking a compassionate and resourceful Medical Social Worker to support our Home Health and Hospice programs.

In this role, you will help patients and families navigate the emotional, social, and financial challenges that come with illness, recovery, and end-of-life care. You will also support coordination of hospice volunteers, helping ensure patients and families receive meaningful companionship and support.

What You’ll Do

• Assess the social, emotional, and financial needs of patients and families

• Develop care plans in collaboration with the interdisciplinary team

• Provide counseling and emotional support to patients and caregivers

• Coordinate access to community resources and support services

• Serve as an advocate for patients and families navigating care options

• Support a patient-centered approach to care

• Assist with coordination of hospice volunteers, including onboarding and communication

• Support volunteer engagement and recognition efforts

What We’re Looking For

• Current Oklahoma LCSW or LMSW license (required)

• Minimum of one year of social work experience in healthcare or hospice preferred

• Experience working with or coordinating volunteers is a plus

• Strong communication and interpersonal skills

• Commitment to compassionate, patient-centered care

• Reliable transportation and valid driver’s license
