Home Health Clinical Services Coordinator (MCP Assistant) – LPN

Working alongside the Manager of Clinical Practice (MCP), Intake, and clinical staff, you'll help coordinate patient admissions, physician orders, specialty programs, and ongoing clinical workflows to ensure patients receive exceptional care from the very beginning of their home health journey.

As a Clinical Services Coordinator, here's a big-picture view of what you'll do: 

  • Support the Manager of Clinical Practice (MCP) and Intake team with daily clinical operations and workflow coordination. 

  • Assist with the referral intake process by reviewing referrals, coordinating admissions, and ensuring timely patient onboarding. 

  • Track, manage, and follow up on physician orders to ensure timely completion and regulatory compliance. 

  • Maintain and monitor the hospital hold list while coordinating communication with hospitals, referral sources, clinicians, and patients regarding hospitalization status and resumption of care. 

  • Conduct patient welcome calls to provide exceptional customer service, answer general questions, and identify any concerns requiring clinical follow-up. 

  • Coordinate and monitor specialty clinical programs and related initiatives. 

  • Monitor clinical reports and communicate outstanding documentation, physician orders, and workflow items to appropriate team members. 

  • Serve as a liaison between patients, caregivers, physicians, referral sources, and agency staff to promote continuity of care. 

  • Assist clinicians in obtaining required documentation and ensuring timely completion of clinical records. 

  • Support compliance with Medicare regulations, agency policies, and documentation standards. 

  • Coordinate clinical meetings, education, reports, and special projects as assigned. 

  • Provide administrative and clinical support for quality improvement initiatives. 

  • Maintain the confidentiality of Protected Health Information (PHI) in accordance with HIPAA regulations. 

Few must-haves for our Clinical Services Coordinator: 

  • Graduate of an accredited Practical Nursing program. 

  • Current, unrestricted Licensed Practical Nurse (LPN) license. 

  • Minimum of two (2) years of nursing experience required. 

  • Two (2) years of recent home health experience preferred. 

  • Experience with physician orders, referral intake, care coordination, or clinical office operations preferred. 

  • Knowledge of Medicare home health regulations and documentation requirements. 

  • Experience using electronic medical record (EMR) systems. 

  • Strong organizational and time management skills with exceptional attention to detail. 

  • Excellent written, verbal, and interpersonal communication skills. 

  • Outstanding customer service skills with the ability to communicate effectively with patients, physicians, referral sources, clinicians, and office staff. 

  • Ability to manage multiple priorities, work independently, and collaborate effectively with an interdisciplinary team. 

  • Proficiency in Microsoft Office Suite and electronic medical record (EMR) systems. 

  • Demonstrated professionalism, flexibility, sound judgment, and commitment to providing exceptional patient care and customer service.
