We are currently seeking LPNs for a private duty case offering up to 40 hours per week, providing consistency, stability, and the opportunity to build a strong connection with your patient.

What You'll Be Doing:

• Provide skilled, in-home nursing care to patients with chronic conditions, post-surgical needs, or other health challenges

• Take vitals, perform assessments, document care, and update the RN supervisor

• Support patients in following their care plan and learning self-care techniques

• Educate family members on care routines, medications, and home safety

• Work alongside a care team including RNs, CNAs, and physicians

• Contribute to discharge planning and ongoing care coordination

What You’ll Need:

• Active Ohio LPN license

• Graduate of an accredited LPN program

• At least 1 year of LPN experience, ideally in home health

• CPR certification

• Familiarity with home health regulations (state and federal)

• Clinical confidence, strong communication, and a compassionate heart
