At Interim Healthcare, we are committed to providing compassionate, patient-centered care that prioritizes dignity and quality of life. We are seeking a dedicated Speech Therapist to support our hospice patients with communication and swallowing needs in a home-based setting.

Responsibilities

• Evaluate patients for speech, language, cognitive, and swallowing disorders
• Develop individualized care plans focused on comfort, safety, and quality of life
• Provide interventions to support communication, cognition, and safe swallowing
• Educate patients, families, and caregivers on techniques and compensatory strategies
• Recommend diet modifications and adaptive equipment as appropriate
• Collaborate with the hospice interdisciplinary team (nursing, social work, chaplain, aides, physicians)
• Accurately document patient care and progress

Qualifications

• Licensed Speech-Language Pathologist (SLP) in Tennessee
• Experience in hospice, home health, geriatrics, or skilled nursing preferred
• Strong knowledge of dysphagia management and communication disorders
• Compassionate, patient-centered approach
• Excellent communication and teamwork skills
• Ability to work independently in a home-based setting
