Interim Healthcare is seeking a compassionate, organized, and proactive CNA to provide hands-on caregiving, develop patient care plans and train Caregivers. This role includes direct patient care with intake responsibilities, helping ensure clients receive the highest quality support from day one. You’ll work flexible caregiver shifts while also collaborating closely with our leadership to meet current and prospective clients in their homes or facilities to create tailored care plans.

 


Direct Caregiver Support

  • Assist with personal care needs like bathing, dressing, and grooming

  • Provide companionship and emotional support throughout the day

  • Prepare meals and perform light housekeeping duties

  • Help with mobility, transfers, and safe transportation

  • Follow care plans and report changes in client condition to supervisors


Client Intake & Onboarding

  • Meet with current and new clients and families in homes or facilities to evaluate and complete patient intake

  • Gather client information and assess care needs in coordination with leadership

  • Write personalized care plans and update records accurately

  • Ensure client care plans are in compliance with agency standards and preferences


Caregiver Coordination & Training

  • Help match caregivers to new clients based on skills, compatibility, and availability

  • Provide on-site or one-on-one caregiver training when necessary

  • Monitor early-phase service delivery to ensure smooth transitions


Compensation

  • $20.50 - $22.00 Per hour dependent on certification and experience

  • Mileage reimbursement for new client visits & quarterly care plan updates

  • Bonuses for new client signups

Experience Requirements

  • Three years of Home Health and Caregiving experience (CNA or similar certification required)

  • Strong communication and interpersonal skills

  • Comfortable working independently in client homes or facilities

  • Organized and capable of managing multiple roles effectively
