Your Role

As our Home Hospice Social Worker, you will deliver outpatient services across patients’ homes, assisted living communities, and skilled nursing facilities. You’ll bring hope and understanding through thoughtful counseling, care coordination, and emotional support—all while honoring the unique path each patient and family travels.

Our Ideal Candidate

• Holds an MSW degree

• Has at least one year of social work experience in a healthcare setting (hospice preferred)

• Understands and embraces the hospice philosophy of care

• Demonstrates strong verbal and written communication skills

• Connects easily with patients, families, and interdisciplinary colleagues

• Possesses a valid driver’s license, auto liability insurance, and reliable transportation

Your Contribution

• Provide counseling and emotional support per physician orders

• Help families navigate resources and develop care or discharge plans

• Evaluate coping mechanisms and address psycho-social needs

• Contribute to bereavement support through individual and group counseling

• Maintain timely and accurate clinical records in line with Inspire Hospice policy
