LPN Monday-Friday (NO HOLIDAYS/NO WEEKENDS) 1:30pm-7:00pm

Join Home Health Services of Virginia as a Licensed Practical Nurse (LPN) working Monday through Friday from 1:30pm to 7:00pm, with no holidays or weekends required. This position is based in Chesterfield, VA, offering a supportive and flexible work environment where you can make a meaningful difference in patients' lives. Responsibilities

• Provide skilled nursing care to patients in their homes according to care plans.
• Monitor patient health and communicate any changes to the healthcare team.
• Administer medications and treatments as prescribed by physicians.
• Document patient information accurately and timely in compliance with company policies and regulations.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Educate patients and caregivers on health management and preventive care.

Requirements

• Valid LPN license in the state of Virginia.
• Current CPR certification.
• Minimum of 1 year experience in home health or a related healthcare setting preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Reliable transportation to patient homes within Chesterfield, VA.
