About the role:

As a ‘Foot Care Nurse’ with Home Concierge, you will be empowering members to receive personalized care in the place that they feel most comfortable, their home, wherever that may be. You will leverage your clinical skills and knowledge to assess, plan and implement foot care with excellence.

The members receiving care are predominantly seniors who want to live and age at home. The care provided is on a per visit basis and you may be required to travel to more than one location during the working day.

  • In collaboration with the healthcare team, the member and their family establish a plan of care based on comprehensive assessment of the member’s needs.

  • Provide ongoing foot care to our members within the scope of the role.

  • Evaluate the plan of care and document the outcome of the care being provided, make appropriate recommendations, and discuss with the broader healthcare team, member, and family.

  • Leverage the provided sterilized, disposable “foot care kit” and ensure safe disposal.

  • Advocate for member rights.

  • Promote member safety.

  • Provide education to member and family around health promotion.

What you need:

  • Current license and in good standing with the CNO.

  • Registered Practical Nurse preferred.

  • Completion of Advanced Foot Care certification.

  • Current ‘G’ Driver’s license, reliable vehicle and insurance.

  • Current Personal Liability Insurance.

  • Experience in community nursing preferred.

  • Ability to problem solve independently.

  • Ability to adopt to new technologies.
