Physical Therapist (Per Diem)

Position Overview:
We are looking for an experienced Physical Therapist to provide evidence-based therapy services in patients’ homes throughout Westport, CT. You will design and implement care plans to help patients regain mobility, manage pain, and return to daily activities safely and confidently.

Key Responsibilities:

• Conduct physical therapy evaluations and establish Plans of Care
• Administer therapeutic interventions tailored to each patient
• Educate patients and families on health maintenance and injury prevention
• Accurately document all patient encounters and progress
• Collaborate with multidisciplinary teams to optimize outcomes

Qualifications:

• Active Physical Therapy license in Connecticut
• Minimum 3 years of experience preferred
• Strong patient rapport and communication skills
• Ability to manage a flexible caseload independently
