Job Summary:
The Home Health Wound Nurse (RN) is responsible for providing specialized wound care services to patients in their homes. This role focuses on assessing, planning, implementing, and evaluating wound care treatment plans in collaboration with physicians and the healthcare team.
The Wound Nurse plays a critical role in promoting healing, preventing complications, and educating patients and caregivers on proper wound care management.
Qualifications
Required Qualifications
- Current Registered Nurse (RN) license in the State of Connecticut
- BLS (Basic Life Support) certification
- Minimum of 1 years of nursing experience in home health or wound care
- Strong clinical knowledge of wound assessment and treatment
Preferred Experience
- Experience managing chronic wounds, pressure ulcers, diabetic foot ulcers, surgical wounds, and wound vacs
- Experience working in a home healthcare setting
- Strong documentation and EHR experience
What You’ll Do
As a Home Health Wound Nurse, you will:
- Conduct comprehensive wound assessments, including evaluation of wound type, stage, and severity
- Develop and implement individualized wound care plans based on physician orders and clinical assessments
- Perform wound care treatments such as dressing changes, topical treatments, and other prescribed interventions
- Monitor wound healing progress and adjust care plans as needed
- Educate patients and caregivers on wound care techniques, infection prevention, and proper home care practices
- Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated care
- Document wound care assessments, treatments, and outcomes in the electronic health record (EHR)
- Monitor for signs of infection and respond appropriately according to physician orders
- Participate in interdisciplinary care discussions and provide updates on patient progress
Compensation
Salary: $100,000 – $110,000 per year
