Job Summary
The Physical Therapist (PT) is responsible for evaluating and treating patients with physical impairments, functional limitations, or mobility challenges. The Physical Therapist develops individualized treatment plans designed to improve strength, balance, coordination, and overall physical function.
This role involves working closely with patients, caregivers, and the interdisciplinary healthcare team to promote recovery and improve quality of life.
Qualifications
Required
- Doctor of Physical Therapy (DPT) or Master’s degree in Physical Therapy
- Current Physical Therapist license in the State of Connecticut
- Current BLS certification
- Strong clinical assessment and treatment planning skills
Preferred
- Experience in home health or rehabilitation settings
- Experience working with geriatric or post-acute care patients
- Strong documentation and patient education skills
What You’ll Do
- Conduct comprehensive physical therapy evaluations for patients
- Develop individualized treatment plans based on patient needs and physician orders
- Provide therapeutic exercises and mobility training
- Monitor patient progress and adjust treatment plans as needed
- Educate patients and caregivers on exercises and safety techniques
- Document patient care and treatment outcomes in the electronic health record
- Collaborate with nurses, physicians, and other healthcare professionals to coordinate care
- Ensure compliance with clinical and regulatory standards
Compensation
Salary: $90,000 – $110,000 per year
